The OwnHealth programme complements and integrates with existing
local healthcare activities to make self care a reality for people
living with long term conditions.
OwnHealth is a pioneering, telephone based, self care
programme for people with long term conditions. OwnHealth is commissioned and
delivered locally and is aligned with the needs, services and
objectives of local general practice, the healthcare community and
the NHS to improve the quality of patient care. It also empowers
patients to self care, improve their health outcomes and reduce the
number of episodes of unscheduled care³. OwnHealth can
also be commissioned directly via the Government Procurement

How does
OwnHealth work?
By creating bespoke care plans - specially
trained healthcare professionals (such as nurses) known as Care
Managers, work with patients with long term conditions to develop
personal care plans. All patients in the programme meet an agreed
selection criteria and have been nominated by their GP. The patient
and Care Manager work together over the phone on a one to one
basis, to identify health related achievable goals and develop an
action plan and tailored care plan that reflects the individual's
circumstances and personal goals.
By empowering patients participating in the
programme to gradually take more responsibility for their own care
through better understanding of their condition, medication and
lifestyle. By using Motivational Interviewing techniques
complemented with tailored educational materials the patient is
supported by the Care Manager to learn more about their condition
and commit to positive lifestyle changes.
By bringing about change over a period of 12 to
18 months patients may 'graduate' from the programme providing both
capacity for new patients and importantly, to each individual
patient the recognition that they have the confidence and knowledge
to manage their own condition more effectively.
