OwnHealth Care Books

The OwnHealth Care books provide the patient with a full
background into their condition. The information content follows
the Basic 8 Self Management Priorities which educate the patient
about their condition and they learn how to improve all aspects of
their self care in an easy to understand way. Not only does this
make the patient think about their condition and gain a better
understanding of it, but the prompts and messages in the books are
motivational and further encourage the patient to consider making
important beneficial changes. The conversations with the Care
Manager are then more meaningful, as the patient has a much better
insight into their condition and what they need to do to manage it
better. The book also includes a handheld record where patients can
keep a track of appointments, medications, reviews, record symptoms
and to record self care goals.
