Patients are central to the whole OwnHealth experience; it is them who make the programme a success.

OwnHealth Care Books
Patients that are enrolled in the OwnHealth programme receive an OwnHealth Care Book. These OwnHealth Care Books are designed not only to help the OwnHealth patient understand more about their condition but also to enable them to take the necessary steps to live with the condition rather than be controlled by it. The OwnHealth Care Book includes the Basic 8 Self Management Priorities to help patients self manage their condition. Those Basic 8 Self Management Priorities are:

The OwnHealth Care books provide the patient with a full background into their condition. The information content follows the Basic 8 Self Management Priorities which educate the patient about their condition and they learn how to improve all aspects of their self care in an easy to understand way. Not only does this make the patient think about their condition and gain a better understanding of it, but the prompts and messages in the books are motivational and further encourage the patient to consider making important beneficial changes. The conversations with the Care Manager are then more meaningful, as the patient has a much better insight into their condition and what they need to do to manage it better. The book also includes a handheld record where patients can keep a track of appointments, medications, reviews, record symptoms and to record self care goals.
