OwnHealth was launched in Nottingham in 2009 and now manages
in excess of 400 patients
Having demonstrated significant outcomes in Birmingham,
OwnHealth was subsequently commissioned by Nottingham City PCT in
July 2009. The service is delivered operationally in a partnership
between NHS Nottingham City, NHS Direct and Pfizer Health
Solutions. The OwnHealth Nottingham service was developed along
similar lines of the highly successful Birmingham
OwnHealth service which is the UK's first large scale
telephone based care management service and in which Pfizer Health
Solution is a partner. The service is provided for Nottingham
residents with long term conditions and focuses on Diabetes and
COPD. Other comorbid diseases such as heart failure and
cardiovascular conditions are also catered for in those individuals
enrolled in the service.
The results of the patients' satisfaction survey indicate high
levels of satisfaction across all the categories which patients
were asked to report on, for example 95% of patients report
improved access to health care services.
These findings are encouraging and serves as a positive
evaluation of the work carried out by the Care Managers.
