CARE (Cancer Awareness and Recovery Enhancement) was launched in
2010 and was commissioned to support 350 patients until March 2011
The Cancer Awareness and Recovery Enhancement (CARE) service was
designed to support women who are completing the active phase of
their breast cancer treatment. The service was made available
to patients who were registered with a GP in NHS Birmingham East
and North and who had completed the active phase of their treatment
at Good Hope Hospital. It provided ongoing support to breast cancer
survivors and their families. CARE was a partnership between NHS
Birmingham East and North and Pfizer Health Solutions and was
delivered by Healthcare at Home, part funded by Macmillan. The aim
of CARE was to align with the Cancer Reform Strategy in that all
cancer patients have a care plan to enable them to move from
patient to survivor and ultimately to reduce hospital follow
up.
CARE service is Pfizer Health Solutions' second project on
breast cancer survivorship. The initial project, 'Surviving Cancer
Living Life', was set up at Guy's and St Thomas' NHS foundation
trust in 2008
