The annual evaluations assess self-reported changes in clinical
metrics, service utilisations and patient satisfaction.
Clinical metrics:
The annual evaluations assess self reported changes in clinical
metrics which are critical to the management of people living with
long term conditions. These metrics include body mass index,
systolic and diastolic blood pressure, cholesterol, high density
lipoprotein, low density lipoprotein and HbA1c. An analysis that
started at baseline indicated that patients in the OwnHealth
programme achieved statistically significant improvements across a
range of clinical metrics³.
Service utilisation:
The annual evaluations also addressed patients' self reported
visits to their GP and to secondary care including emergency
department and outpatients attendances. The data analysis is based
on records entered by Care Managers in the OwnHealth IT system
during their telephone conversation with patients. Year on year
comparisons have demonstrated reductions in service utilisations
across primary and secondary care.
Patient satisfaction:
The service delivery model undergoes rigorous evaluation to
understand the level of patient satisfaction. Specifically,
patients are asked to evaluate their ability to self care; their
reduced need to attend hospital; improved access to services; and
whether telephone based care is an acceptable and effective way of
communicating and overall satisfaction with the service provided.
Since the programme inception positive outcomes have been
established in all measured areas³.
