What does the module contain?

  • Introduction and objectives:
    provides an overview of the OwnHealth programme; the OwnHealth Choice suite of modules; information on how to use the guide, the purpose of OwnHealth and the disease areas covered and outlines the aims and objectives for the programme.
  • Programme design:
    includes guidelines on selecting appropriate long term conditions for OwnHealth in your area; planning operational work streams and project governance; building a timeline for launch; contractual and service model agreements; and reporting and accountability.
  • Implementation:
    provides instructions on planning, building and training your team; the OwnHealth Choice Self Care Coaching Course and OwnHealth Long Term Condition e-learning modules; continuing professional development; rewarding team success and project management systems. This section also outlines building patient evaluations systems; patient monitoring and recording; managing risk and issues, and infrastructure essentials.
  • Project management:
    includes procedure on supporting and monitoring members; selecting patients; launch mapping; launch promotion and planning for expansion.
  • Evaluation and outputs:
