The OwnHealth Choice long term conditions modules provide distance
e-learning training on clinical and management guidelines within
key disease areas.
The eight hours of learning from the OwnHealth Choice Long
Term Condition Modules is delivered in a unique format, to support
clinical staff to understand the disease area and to enhance their
skills in encouraging their patients to self care.
The modules provide clinical and background information, and
case studies using Motivational Interviewing techniques to patients
with the long term condition that the module is based on. The
modules currently available are:
- Type 1 and Type 2 Diabetes
- Chronic obstructive pulmonary disease
(COPD)
- Cardiovascular disease (CVD)
By the end of the module, delegates will be:
- Aware of the latest clinical guidance
within specific disease areas
- Able to understand how Motivational
Interviewing techniques apply to particular long term
conditions
- Able to support patients' self care
within specific disease areas
- Equipped with materials proven to
support educational, attitudinal and behavioural changes in
patients
