The patient is a 69 year old woman who has been on the
OwnHealth programme since 2008; she has a diagnosis of COPD
and essential hypertension. Her current Care Manager has been
working with her since May 2010. She was recently discharged
from hospital after being informed by the consultant that she has
infective COPD.
When asked about her care, she informed the consultant that she
sees the practice nurse once every 12 months for her COPD and
receives a call from the OwnHealth Care Manager every
month. The consultant wanted to send a summary report about
the patient to the practice nurse, the patient requested that it
also be sent to her Care Manager at OwnHealth.
She said
My Care Manager is my rock she always explains everything in
simple language to me."
