What is care management?
The basic principle of care management is to support and empower
individuals to make health behaviour changes that have a positive
effect on their health and improve their well-being.
Care management is delivered by Care Managers who are highly
experienced oncology nurses who provide clinical information and
support to breast cancer survivors. The Care Managers take a
holistic, patient-centred approach that supports the patients
recovery path. The Care Manager also works closely with the
multi-disciplinary cancer teams based at the Guy's and St Thomas'
Foundation Trust.

Who are the care
managers?
The Surviving Cancer Living Life Care Managers are
experienced oncology nurses who undertake a robust trained
programme that includes the principles of Care Management. They
also attend the two day interactive classroom based
OwnHealth Self Care Coaching Course where they are trained in
Motivational Interviewing techniques and includes the use of actors
to support the learning of the Care Managers.

The role of the Care Manager includes advising the patient about
different support services including Surviving Cancer Living
Life whilst they are still receiving treatment. This process
enables the patient to make informed decisions about which support
service they wish to join after active treatment, the Care Manager
then refers the patient appropriately.
