The Birmingham OwnHealth® programme is a collaboration between Birmingham East and North Primary Care Trust, NHS Direct and Pfizer Health Solutions

The service provides pro-active telephone-based coaching to Birmingham residents with long term conditions in the most deprived communities

The Care Managers employed by NHS Direct take a holistic, not just disease based approach to care and are supported by bespoke software that is adapted to incorporate local guidelines and pathways
•
The programme will be supporting up to 10,000 people by the end of 2008
•
The long term conditions covered are diabetes, cardiovascular disease, heart failure and COPD
•
The service is currently offered in English, Punjabi, Urdu and Gujarati
